Tackling Transportation Challenges to Advance Health Equity

Connect2Health at the 11th Annual Population Health Innovations Summit

​

I was truly honored to be invited as the youngest speaker at the 11th Annual Population Health Innovations Summit: SDoH and Health-Related Social Needs to Improve Outcomes for All. During my session, I addressed a critical yet often overlooked barrier to healthcare access: transportation.

​

I shared how health plans, providers, and community-based organizations (CBOs) are implementing innovative strategies, building cross-sector partnerships, and strengthening communication to tackle transportation challenges that disproportionately impact underserved, underinsured, and under-resourced communities.

​

It was an inspiring experience to engage with leaders from health plans, physician groups, hospitals, Accountable Care Organizations (ACOs), integrated delivery systems, government agencies, and CBOs—and to be part of such a meaningful and solution-driven conversation.

​

Being part of this summit gave me the opportunity to advocate for practical, scalable solutions and to further elevate the mission of Connect2Health—closing health care disparities and promoting equity through action and collaboration.

We are incredibly proud to recognize our co-founder, Krish Tyagi, for being awarded the Student Champion (K-6) Award by School Connect in 2025. At such a young age, Krish has been pivotal in leading Connect2Health's initiatives, driven by his deep compassion and commitment to making a difference in the lives of those in need. His leadership shines through our outreach programs, where he actively contributes to the packing and distribution of essential resources, including Sick Kits, Wellness Kits, Kindness Kits, Emergency Food Boxes, and Naloxone Harm Reduction Kits. Krish's selfless dedication to supporting homeless and underserved communities serves as an inspiring reminder of the impact that empathy and action can have, regardless of age.

This esteemed award, established by President George H.W. Bush in 1989, celebrates individuals and organizations whose volunteer efforts create meaningful, lasting impacts on the lives of others. To be acknowledged among such remarkable changemakers is both a profound privilege and testament to our mission.
 

From the very start, Connect2Health has been driven by the belief that access to quality healthcare is a fundamental human right, not a privilege. Our volunteers work tirelessly to deliver critical resources—essential medical supplies, hygiene kits, wound care kits, and nutritious food boxes—to populations often overlooked and underserved.  The recognition by Points of Light highlights the heart and purpose behind our mission: providing care, hope, and essential services to those who need it most.

We extend our heartfelt thanks to Points of Light for this tremendous honor and to our volunteers and supporters for their unwavering commitment to making a difference.

We’re truly honored to be featured in the McDowell Mountain Ranch newsletter for September and October, highlighting our assembly of 1,002 naloxone kits for opioid overdose prevention. This recognition reflects our commitment to addressing substance use in our community. Each kit represents a chance to save lives and promote awareness.
