1. Outpatient Care

​

Integrated outpatient healthcare for those facing homelessness in Maricopa County. Circle the City has two outpatient clinics that provide primary and preventive healthcare. They support both walk-ins and scheduled appointments. Outpatient care, also known as ambulatory care, is defined as a service or treatment that does not need to be conducted in a hospital setting or does not require an overnight stay following a procedure.

Services Include​: Medical evaluation of illness or injury, Preventive care, Evaluation and management of chronic diseases, Immunizations, including COVID-19 and seasonal flu vaccine, Behavioral health and substance use consultation, Case management including assistance with IDs, social services, and housing, Transportation assistance, Care coordination, Pediatric care including well-child visits at Parsons Family Health Center.

2. Respite Care

Integrated outpatient healthcare for those facing homelessness in Maricopa County. Circle the City has two 50-bed medical respite centers for adults recovering from acute illnesses or injuries. Respite care is for people who are not sick enough to be in the hospital, but too sick to be on the street.

3. Street Medicine

Delivering healthcare to unhoused individuals in river bottoms, canals, alleys, street corners, parks and other areas. Circle the City’s Street Medicine Outreach engages unhoused individuals who do not readily access primary healthcare services in the community by delivering this care in river bottoms, canals, alleys, street corners, parks and other areas. Street medicine care is transitional by connecting patients to comprehensive medical and behavioral health services.

The health navigator works with hospital staff to connect patients facing homelessness to support services upon discharge.
